For eligible customers, simply download the claim form that will be available here from Friday 5th November, or alternatively customers can call Panasonic Customer Care on 132 600 for a claim form.
Once the claim form is completed and mailed in with the original purchase receipt, it is validated and a copy of AVATAR on 3D Blu-ray will then be mailed to you at no additional charge. If you do not have your original purchase receipt, you will need to contact your retailer for a reprint.
